Community Outreach Program – Purpose
The program was developed to strengthen the quality of connection between patients and family doctors so that primary care treatment is optimized and more efficient, thus reducing the burden on GPs and improving the patient-provider experience. Connecting clients to the appropriate resources and alleviating the psychosocial challenges assists the doctors and patients to have a more medically centered, quality consultation.
The services provided are based on the client’s needs, and may include assistance with applications for housing, Income Assistance, Persons with Disabilities designations, accompaniment to doctor’s appointments, and other basic life skills supports which enable health care to become manageable.
Who can Participate
This program is by physician referral. People seeking assistance through this program should contact their doctor to ask for a referral.
